This year, 15 eligible children’s charities will be selected through a random drawing to be a part of the 15th Anniversary of Miracle on Canal Street. In addition, Potawatomi Bingo Casino will host an Alumni Second-Chance Drawing for qualified alumni charities. One alumni charity will be drawn.
A new feature added this year is the Mega Miracle charity, which will be selected by Potawatomi Bingo Casino management to receive approximately 20% of the funds generated. To be eligible for the Mega Miracle, you must fill out the specific Mega Miracle questions.
To apply for the chance to be chosen as a 2008 Miracle on Canal Street charity, go through the application process thoroughly. Please complete each step to ensure all components are submitted with your application. Your completed application must be received by 4 p.m. (CDT) on Friday, June 27, 2008.

View the Submission Guidelines to determine if your charity is eligible for Miracle on Canal Street (includes eligible organizations, ineligible organizations and additional qualifiers and restrictions). |
Step 2: |
Download the application, save it to your computer and fill it out completely by typing directly into the document. Ensure all fields are completed, as missing or incorrect information could lead to ineligibility. Hand written applications will not be accepted. Print the application once completed. |
Step 3: |
Attach the following required forms to your application:
• Copy of the IRS Federal Tax Exemption Determination Letter
• Copy of the most recent IRS Form 990
• Completed W-9 Taxpayer Identification Number and Certification Form
(included with the application)
• Most recent audit and management letter |

Please ensure that all information from Steps 2 and 3 is included and send your application as outlined. |
